The high levels found in obese OSA patients could be an expression of obesity itself rather than an expression of OSA severity. We compared data of OSA without obesity (Group 1,n=29,age 55.3±12.8,AHI 55.6±25.4,BMI 25.3±3.1), those ones of obese OSA (Group 2,n=46 age 55.3±12.8,AHI 55.6±25.4,BMI 42.8±8.6), and of obese without OSA patients (Group 3,n=10,age 36.4±12.5,AHI 7.7±2.3,BMI 45.3±7.5). MCP-1 did not differ in the 3 groups (406.8±205.8,391.4±130.5,360.9±171 pg/ml, respectively); no correlations were found in all patients between MCP-1 and age, BMI or sex. In contrast, we found a statistically significantly correlation between MCP-1 and ODI (r=0.4,p=0.036),T90 (r=0.54,p=0.003),mean amplitude of desaturation (r=0.68,p<0.001) only in Group 1.Patients were classified according to MCP-1 distribution: MCP-1<489.7 or ›489.7 pg/ml corresponding to upper quartile. Those in the upper quartile show more severe sleep hypoxia (T90 10.4±12 vs 2.6±3.8%,p=0.01) and mean amplitude of desaturation (5.9±2.2 vs 4.2±0.6 %,p=0.005) in comparison with those ones in lowers quartiles.A backward stepwise regression model showed that higher values of MCP-1 were predicted (R=0.41,p=0.007) by the mean desaturation amplitude (B=0.3,IC 0.08-0.5) and by the OSA severity (B=0.4,IC 0.1-0.8). Our data suggest an activation of MCP-1 pathway in OSA patients, independently of obesity, strictly correlated with hypoxia severity. %U
