The commonest cause is sleepiness.Aim Estimation of independent predictors of SDB and its cut off points in Egyptian commercial drivers.Patients and Methods This is a cross-sectional descriptive study followed by a nested case-control study was conducted at chest department- Mansoura University in the period of Jan 2014 till Dec 2015. It includes 110 male commercial drivers self-referred with symptoms suggesting sleep disordered breathing. Data collected included: smoking, age, body mass index, neck circumference, Epworth sleepiness scale (ESS), Functional outcome of sleep questionnaire (FOSQ), Berlin questionnaire, STOP Bang questionnaire, Friedman OSAS score, modified mallampati score, full night polysomnography. We found 90 drivers were with apnea hypopnea index (AHI) ≥ 5 and divided them into two groups (with and without SDB).Results The prevalence of SDB was 81.8%. significant differences in comparison of the two groups of drivers were found as regard neck circumference (p = 0.003), Berlin questionnaire, STOP Bang score, modified mallampati score, Friedman OSAS diagnostic score, ESS, FOSQ (p ≤ 0.001 for all 6 variables). In Multivariate analysis, we found two independent predictors(with percentage for accuracy of this model= 92.7%): NC, ESS (OR = 1.3, 1.8 respectively).the calculated cut off points of ESS was (7) with sensitivity 91.5% and 80% specificity, NC cut off point was (39.5) with sensitivity 88.1% and 50% specificity.Conclusion We recommend that ESS ≥ 7 plus NC ≥ 39.5cm are independent clinical predictors for SDB, these cut off points can be applied in routine medical examination of license renewal for detection of SDB risky drivers.
