    A patient education activation card. This card was developed for use by health care providers at National Jewish Health (NJH) to assess care of their patients with idiopathic pulmonary fibrosis (IPF). Metrics for IPF care were selected using clinical recommendations and developed in collaboration with a multidisciplinary clinic team.
